New Patient Pathway

 

If you are a prospective bariatric patient, i.e. a patient looking to undergo a bariatric or metabolic procedure in our surgical services, please read the following and follow the instructions carefully.

If you have not made your initial appointment with our office, please call 818-848-8311 and request an initial bariatric consultation.

Before you come to our office, please download, print, and complete the following Forms. If possible, fax or email these forms to registration@transmed.mobi.

THE INITIAL APPOINTMENT

 

​

 

This is a lengthy appointment that will last approximately 3 hours.

​

If you have not faxed or emailed the completed forms [F1, F2], please bring them to the office for your appointment.

 

If you were previously seen in our offices, centers, or another COE Bariatric Center, bring all documentation or reports you have from these consultations.

 

Your demographic and health information will be entered into our Electronic Health Record System. 

 

You will meet one of our intake staff [one-on-one] for a quick screening.

 

You will be asked to attend a 90 minutes Educational Group Meeting and/or video presentation [enhanced COVID-19 protocol]. At that time, you will be given an educational or reference manual to take home.

Please note, most of our intake group sessions have been suspended during the COVID Pandemic.

 

You will then have a consultation with the clinical team and meet your surgeon.

​

At the end of this appointment, should you wish to proceed, you will be entered into our qualification program.

 

Finally, your success, safety, and health are of paramount importance. Please, follow all instructions carefully and become an educated and informed patient.
